Support at Home

• Evaluate needs for care and for social and emotional support
• Advise about home care and home care providers
• Facilitate arrangements needed to make a home safe and comfortable

Advise and Help When a More Supportive Living Situation is Needed

• Provide advice and guidance about independent living facilities, assisted living facilities, and nursing homes. Manage and advocate for care related to such facilities.
• Evaluate when a move for care is necessary, oversee and arrange for that move

Government Programs

• Work with families regarding the availability of government programs, such as Medicaid, that can help pay for long-term home care and nursing home care.

Coordinate Medical Appointments and Other Services, Ensure Communication with and Among Loved Ones

• Accompany clients to visits with physicians and other helping professionals
• Follow up on healthcare provider recommendations for equipment, services or therapies, and ensure that they are implemented
• Provide ongoing updates to the “team” of loved ones and ensure that the flow of information and effective communication are consistently maintained

Financial Management

• Financial management and bill paying services to ensure that expenses including rent, utilities, and fees to providers and vendors are paid in a timely manner
